Methodology

How Healthcare Wiki researches, sources and updates its articles, guides, payer profiles and tools.

Sources

We work from primary sources wherever they exist: CMS rules and guidance, state Medicaid agencies, departments of insurance, statutes, and published payer policy bulletins and provider manuals. Secondary reporting is labeled as such and linked.

Rankings and tools

Any ranking, benchmark or calculator we publish will show its factors, data sources and last update date on the page itself. Calculators show their assumptions so you can adjust them for your own situation.

Update dates

Every article shows when it was published and when it was last updated. Payer processes and regulations change often, so treat anything older than 12 months as a starting point and confirm with the payer or agency.

Limitations

Our content describes typical rules and experience. It is not a guarantee for any single application, contract or claim, and it is not legal advice.
