Health Outcome Survey (HOS)

Health Outcome Survey (HOS) is an annual patient‐reported outcome survey, conducted by a contracted Centers for Medicare & Medicaid Services (CMS) vendor for Medicare Advantage plans. The goal of the survey is to gather valid and reliable health status data for use in quality improvement activities, public reporting, Medicare Advantage Organization accountability and improving health outcomes. The survey contains questions regarding physical and mental health, chronic medical conditions, functional status (e.g., activities of daily living), clinical measures and other health status indicators. Six of the survey areas are included in the CMS Star quality measures
